It is essential for all students to be on time to their scheduled training, if students are going to be late, they are required to contact the office or their instructor. Arrangements will be made in order to make up the lost time. Students will be removed from the roster if they have more than three unexcused absences or if they are late more than four times. If by any reason the student is dropped from the course, they must meet with the director. Not adhering to the school’s rules, regulations, policies, and code of conduct 2. Missing more than 20 percent of instruction time 3. Not maintaining the minimum grade point average 4. Not meeting financial responsibilities to the school The school director will notify the student in writing should it become necessary to dismiss the student. The dismissal letter will contain the date and the reason for dismissal. Any student that is dismissed from the course for any reason may appeal in writing to the academy in order to be reconsidered for reinstatement. I agree to the religious accommodations Cancellation and Refund PolicyCancellation and Refund Policy(Required)1. The school must refund all money paid if the applicant is not accepted. This includes instances where a starting class is canceled by the school. 2. The school must refund all money paid if the applicant cancels within five business days (excluding Sundays and holidays) after the day the contract is signed, or an initial payment is made, as long as the applicant has not begun training. 3. The school may retain an established registration fee equal to ten percent of the total tuition cost, or one hundred dollars, whichever is less, if the applicant cancels after the fifth business day after signing the contract or making an initial payment. A “registration fee” is any fee charged by a school to process student applications and establish a student record system. 4. If training is terminated after the student enters classes, the school may retainthe registration fee established under (3) of this subsection, plus a percentage of the total tuition as described in the following: - If the student completes one week or up to 10%, whichever is less, the school may keep 10% of the tuition cost. - If the student completes more than one week or 10% whichever is less but less than 25%, the school may keep 25% of the tuition cost. - If the student completes 25% through 50%, the school may keep 50% of the tuition cost. - If the student completes more than 50%, the school may keep 100% of the tuition cost. 5. In calculating refunds, the official date of a student’s termination is the last day of recorded 6. Attendance (a) When the school receives notice of the students intention to discontinue the training program; or (b) When the student is terminated for a violation of a published school policy that provides for termination; or, (c) When a student, without notice, fails to attend classes for thirty calendar days. 7. All refunds must be paid within thirty calendar days of the student’s official termination date. I agree to the cancellation and refund policy Alcohol and Drug Abuse PolicyAlcohol and Drug Policy Statement of Purpose and Policy(Required)Drivers are an extremely valuable resource for H & R Elite Trucking Academy business. Their health and safety are a serious company concern. Drug or alcohol use may pose a serious threat to driver health and safety. It is the policy of the company to prevent substance use or abuse from having an adverse effect on our drivers. The company maintains the work environment safer and more productive without the presence of alcohol, illegal or inappropriate drugs in the body or on company property. Furthermore, drivers have a right to work in an alcohol and drug-free environment and to work with drivers free from the effects of alcohol and drugs. Drivers who abuse alcohol or use drugs are a danger to themselves, their coworkers, and the company’s assets. The adverse impact of substance abuse by drivers has been recognized by the federal government. The Federal Highway Administration (FHWA) has issued regulations which require the company to implement a controlled substance testing program. The Company will comply with these regulations and is committed to maintaining a drug-free workplace. All drivers are advised that remaining drug free and medically qualified to drive are conditions of continued employment with the Company. Specifically, it is the policy of the Company that the use, sale, purchase, transfer, possession or presence in one’s system of any controlled substance (except medically prescribed drugs) by any driver while on company premises, engaged in company business, while operating company equipment, or while under the authority of the Company is strictly prohibited. FHWA states that mandatory testing must apply to every person who operates a commercial motor vehicle in interstate or intrastate commerce and is subject to the CDL licensing requirement. The execution and enforcement of this policy will follow set procedures to screen body fluids (urinalyses), conduct breath testing, and/or search all driver applicants for alcohol and drug use, and those drivers suspected of violating this policy who are involved in a US Department of Transportation (DOT) reportable accident or who are periodically or randomly elected pursuant to these procedures. These procedures are designed not only to detect violations of this policy, but also to ensure fairness to each driver. Every effort will be made to maintain the dignity of drivers or driver applicants involved. Disciplinary action will, however, be taken as necessary. Neither the policy nor any of its terms are intended to create a contract of employment or to contain the terms of any contract of employment. The Company retains the sole right to change, amend or modify any term or provision of this policy without notice. This policy is effective 10/26/2012 and will supersede all prior policies and statements relating to alcohol or drugs.
